New Patient Forms
An ophthalmic assistant will perform your preliminary examination. The assistant will ask several questions pertaining to your visual health, including the reason for your visit, a thorough medical and family history, current medications, and allergies. A variety of tests will be administered to measure your vision and the general health of your eyes. Upon completion of the preliminary testing, drops will be placed in your eyes to dilate your pupils. After approximately 20 to 30 minutes, your eyes will be fully dilated, the doctor will utilize a high powered microscope to examine the front and back of your eye.
Results of the examination and any treatment that may be needed to obtain the best possible vision will be discussed with you at that time. An initial examination requires approximately one hour. Some patients experience difficulty driving after dilation due to light sensitivity. You may want to prepare in advance for a driver to accompany you. Sunglasses are recommended and will be provided if you do not have a pair.
We encourage you to fill out your new patient form prior to your visit. The form is available in a convenient PDF format. If you need to download Adobe Acrobat reader, it’s available free by clicking on the appropriate link on the right side of this screen to download our patient forms.
